How to apply for a leave?

1. Download the application form at the website of Student Assistance Division, then, click the Application Form, menu (left side).

2. Please hand out through Course Teacher, Room Teacher and the chair or chairman.

How to apply for a group insurance?

1. Any insured who suffered from illness or accident could claim the hospital room and board insurance.

2. Attached with:

Medical certificate (reserved copy)

The expense receipt principal edition and the expense are detailed (reserved or duplicate copy)

personal bankbook (duplicate copy)

How to obtain an application form for a group insurance?

Please contact the clerk at Room 5008, Administration Building, and fill out the application form.

Download the application form at the website of Student Assistance Division, then click the Group Insurance, menu (left side).

How long should propose a valid application after the accident happen?

Once occurred, to apply for a claims settlement, no late than 2 years.

Could foreign students apply for scholarship? What is it?

Student Assistance Division, NSYSU is only responsible for Taiwanese students’ affaires. On the issue of non-Taiwanese students, please contact Division of Overseas and International Degree Student Affairs, 2F of Administration Building.
